"THYROID MASS"

Clinical history: 41y.o. woman presents with a 10 cm. mass which surround completely the thyroid (pict. 1); no FNA of the lesion is performed and the mass is surgically removed.
Microscopic description: the neoplasia is mainly on the outer surface of the thyroid from where it infiltrates the parenchyma (pict. 2).
The lesion has mainly epithelioid features with clear cells (pict. 3) - (pict. 4), squamous-like cells (pict. 5) - (pict. 6) and more undifferentiated areas with markedly atypical cells in a fibrous stroma (pict. 7) - (pict. 8).
The clear cells are negative with immunohistochemical staining with antisera for CD5.
In some of the sections in the peri-capsular region, rests of normal thymus (pict. 9) - (pict. 10) are noted.
Mitotic count is low and there is neoplastic vascular invasion.
