D. RICCI - F. FEDELI - La Spezia Hospital
(pictures by dr. Fedeli)

Clinical history: a 30y.o. man presents with a well defined, 0.6 cm, skin papule on the forehead since indefinite time.
Microscopic description: the lesion consists of a well defined but not encapsulated proliferation of irregular lobules of epithelial cells (pict 1) - (pict 2).
At the periphery of the nests there are two or three layers of basaloid-type cells with the typical palisading feature and in the center there is a chore of lymphocytes and histiocytes (pict 3) - (pict 4).
The small lymphocytes show mixed B and T cell phenotype (pict 5) - (pict 6).
The largest cells have pale cytoplasm and prominent nucleolus and show immunohistochemical positivity for protein S-100 (pict 7).
There is no cytological atypia. The lobules are surrounded by fibrous stroma and lymphocytes.
