Clinical history: 47y.o. woman during colposcopy presents a cervical polyp on which a biopsy is performed. Following the biopsy diagnosis a total hysterectomy is performed.
Macroscopic description: the cervix shows a 5 cm polyp (pict. 1) which occupies the entire endocervical canal.
Microscopic description: the lesion is relatively well defined and consists of a solid proliferation of atypical , even if monomorphic, spindled cells which show a high mitotic index (7/HPF) (pict. 2) - (pict. 3) - (pict. 4).
At the periphery of the lesion, towards the muscular layer the tumor cells show large cytoplasm and rather atypical nuclei (pict. 5) - (pict. 6) - (pict. 7).
There are also sparse micro-cystic spaces, lined by cuboidal epithelium with polypoid projections into the lumens (pict. 8) - (pict. 9).
The neoplastic cells are positive with immunocytochemical staining for Vimentin and focally for protein S-100, in particular around the pseudo-cystic areas (pict. 10) - (pict. 11); immunocytochemical staining with antisera for Actin and Desmin are negative.
Staining for Estrogen receptors is positive in the epithelial cells nuclei while the neoplastic cells are negative (pict. 12).
