"ACUTE ABDOMEN IN A 41Y.O. MAN"

Clinical history: A 41y.o. man undergoes emergency surgery for acute abdomen.
A large peripancreatic mass with multiple peritoneal nodules and a small liver nodule are found. A diagnostic biopsy is performed.
Microscopic description: there is a diffuse proliferation of isolated large atypical cells with abundant cytoplasm and vesicular nuclei with prominent nucleoli (pict. 1) - (pict. 2) - (pict. 3).
Islands of normal mesothelium are present in between the neoplastic cells (pict. 4) and necrotic areas are also seen.
The neoplastic cells are strongly positive with immunohistochemical staining for Vimentin (pict. 5) and isolated cells are positive for Calretinin (pict. 6).
Staining for EMA, Cytokeratin (pict. 7), CD45 (pict. 8), CD20, CD45RO, Kappa and lambda chains, S100 and HMB45 are negative.
