"CERVICAL MASS"

Clinical history: 80y.o.woman presents with a deeply fixed cervical mass.
Macroscopic description: 3.8x3x2.8 cm ovoid mass, which on cut section, is white-greyish with small hemorrhagic areas.
Microscopic description: lymph node parenchyma (pict. 1) - (pict. 2) is still present at the periphery of the lesion.
This consists of a proliferation of loosely attached, irregularly shaped cells with large cytoplasm (pict. 3) - (pict. 4).
In some areas there is a storiform pattern (pict. 5).
Nuclei have often an intra-nuclear vacuole (pict. 6).
Mitotic index is relatively low (pict. 7).
The lesion has pushing margins. The residual lymph node parenchyma shows hyperplasia. The neoplastic cells are positive with immunohistochemical staining for CD21 (pict. 8) and for HLA DR (pict. 10) and negative for CD3 (pict. 9) and CD20.
