Clinical history: a 43y.o. woman presents with a vulvar labium majus lesion and bilateral regional lymph nodes enlargement. A biopsy is performed and after that a vulvectomy and lymphadenectomy is done.
Microscopic description: sections of the biopsy show epidermis ulceration and Paget-like alterations (pict. 1) - (pict. 2).
In the surgical specimen there are still Paget –like features but also extensive neoplastic infiltration of the sub-epithelial stroma (pict. 3); the neoplastic cells form nests and cords admixed to abundant mucus (pict. 4) - (pict. 5), sometimes with signet ring cell features (pict. 6).
Inguinal lymph nodes have the same appearance.
