"THYROID NODULE"

Clinical history: a 79y.o. woman with no previous clinical history presents with a “toxic goitre”.
During surgery for thyroidectomy the surgeon requests a frozen section but it is not performed because there is no evidence of macroscopically suspicious lesions and because of the presence of calcified nodules.
Macroscopic description: a multi-nodular thyroid is observed, with the largest nodule, measuring 6 cm in diameter, in the right lobe.
Microscopic description: The right nodule consists of cords of clear cells, sometimes with granular cytoplasm, round nuclei and prominent nucleoli; the lesion is infiltrating the surrounding tissue ( (pict. 1) - (pict. 2) - (pict. 3)).
Necrosis and haemorrage are present ( (pict. 4)), with haemosiderin deposits ( (pict. 5)).
Immunohistochemical staining for thyroglobulin ( (pict. 6)) is negative.
