"THYROID MASS"

Clinical history: in November 2000 a 54y.o. man presents with a rapidly growing mass in the thyroid left lobe and isthmus, which infiltrates the surrounding tissue. A chest CT scan is negative. An abdominal ultrasound reveals a solitary, 3 cm, liver nodule. A thyroid FNA and a liver needle biopsy are performed.
Microscopic description of the cytological specimen: the smear is very cellular and shows pleomorphic elements, with abundant cytoplasm, irregular nuclei and prominent nucleoli (pict. 1) - (pict. 2).
Microscopic description of the liver biopsy: the liver parenchyma is completely replaced by large spindled cell, with irregular nuclei and prominent nucleoli. Mitoses are frequent (pict. 3) - (pict. 4). These cells are positive for cam 5.2 Cytokeratin, (pict. 5)
and for Laminin (pict. 6)
and they are negative for EMA, S100, HMB 45, CD 34, and CD31.
