"CUSHING SYNDROME"
B. E. LEONE - Ospedale San Raffaele - Milano
(pict. by dr. Faravelli)
Clinical history: a 23y.o.ma presents with the classical clinical features of the Cushing syndrome, confirmed by the haematological clinical examinations.
An abdominal ultrasound shows normal adrenal glands and the sella turcica is also of normal appearance.
The patient undergoes surgery for the removal of both the adrenal glands, but for technical problems during the surgical operation,,ly the right adrenal is removed.
Macroscopically the surgical specimen has normal appearance.
Microscopical description: sections show intra-cortical, sometimes confluent, 1-2mm micro-nodules ( pict. 1 - pict. 2 - pict. 3), which consist of larger cells than the normal cortical,es (left, pict. 4).
They have eosinophilic ( pict. 5 - pict. 6) or micro-vacuolated ( pict. 7) cytoplasm; the nuclei are relatively small and regular. In the cytoplasm there is often a brownish pigment, likely referred to lipofuscin ( pict. 8 - pict. 9).
