"PLEURAL LESION"

Clinical history: a 60y.o. man presents with repeated, mono-lateral pleural effusion and a plaque-like lesion of the left visceral pleura. He did not have work exposure to asbestos but he is coming from a geographical area with an increased incidence of mesothelioma.
The clinical blood examinations are all negative as well as repeated cytological examinations, the sputum. X-ray and CT scan of the lungs show no other lesions. A biopsy under thoracoscopy is performed.
Microscopic description: sections show a diffuse proliferation of spindled cells with marked atypia (pict. 1 - pict. 2 - pict. 3 - pict. 4 - pict. 5).
The immunohistochemical staining show a diffuse and strong positivity of the neoplastic cells for Cytokeratin Pool (pict. 6), Cytocheratin 7 (pict. 7), Vimentin (pict. 8) and BerEP4 (pict. 9); EMA, CEA and Calretinin are weakly positive; CD15 and Cytokeratin 20 are negative.
