(pict. by dr. Fiaccavento)

Clinical history: a 56y.o.woman presents with a 3 cm left cervical nodule. The patient refers a partial thyroidectomy in 1998, in other hospital, for a follicular adenoma with central hemorrhagic pseudocyst.
An US guided FNA is performed and then a cervical lymphadenectomy is done.
Cytological description: according to the features shown in the pictures 1,2 and 3 the diagnosis of malignant neoplasia, suggestive of metastatic disease, is done.
Microscopical description: the lymph node architecture is largely replaced by neoplastic cells with large cytoplasm and atypical nuclei (pict. 4 - pict. 5 - pict. 6).
With immunohistochemical staining the neoplastic cells are negative for LCA (pict. 7) and for Lysozime (pict. 8) and are strongly positive for CD21 (pict. 9) and CD23 (pict. 10).
One of the cytological smears has been stained later for CD23 (pict. 11) and is strongly positive.
