(pict. by dr. Carinelli)
Clinical history: 70y.o. woman after an ultrasound diagnosis of right ovarian cysts, undergoes isterectomy with bilateral adnexectomy.
Macroscopical description: 8x3.5x2 cm uterus with 1cm, fibrotic, left ovary and 4 cm long left tube. The right ovary shows an 8 cm cyst with smooth and shiny, whitish wall. The right tube measures 7 cm in length.
Microscopical description: the wall of the cyst consists of fibro-vascular tissue with numerous smooth muscle fibres and numerous small and medium sized arterioles and venules (pict. 1).
Focally ovarian stroma is still recognizable. The lining epithelium is monostratified cubical (pict. 2), costituito da cellule con citoplasma eosinofilo senza evidenti cilia e formava diverse invaginazioni con indentature. pict. 2
and forms indentation; the cells have eosinophilic cytoplasm without cilia. In the wall of the cyst there are also tubules and small cysts lined by rete ovarii like cells. There are also numerous sub epithelial nests of hilar cells in the cyst wall and around the other tubules (pict. 3 - pict. 4).
Some of these cells show the typical Reinke crystals..
